Alter Life Sciences LLC. d/b/a Care PredictorTM has prepared this Privacy Policy to inform you of our privacy practices with respect to the personal information we collect in connection with the use of our Care PredictorTM Index (“CPI”). By using the CPI, you agree to the collection and use of information in accordance with this Privacy Policy. INFORMATION COLLECTION

From visitors to the CPI website, [https://carepredictor.com/], we collect personal information to the extent you provide it through a webform, such as first and last name, email address, job title and company.

From our employer customers, we collect personal information such as first and last name, email address, phone number, address, other business contact information, username and password, and payment information. 

From respondents to our CPI assessment, we collect personal information such as first and last name, email address, username and password, along with any personal information that may be contained in answers to questions posed in the assessment. 

From visitors to the CPI website, customers, and respondents, we may also collect certain usage or network activity information such as data about the device used to access the CPI website or the CPI assessment, IP address, browser type and settings, referring web page, pages visited, and cookies. See the COOKIES AND TRACKING TECHNOLOGIES section below for more information. 

HOW WE USE INFORMATION 

Our main purpose for collecting information from you is to facilitate use of the CPI, including to provide assessment score(s), assessment report(s), and/or other reports derived from the assessments such as those that aggregate scores of multiple respondents.  

Additionally, Care PredictorTM may use the information we collect pursuant to this policy, including personal information, for the following purposes: 

  • To create and manage your account with us
  • To provide you with customer support
  • To communicate with you and respond to your inquiries
  • To provide information you have requested
  • To provide information or advertisements that may be of interest to you
  • To administer, personalize, and improve our services
  • To develop new features and services
  • To generate statistics and de-identified data
  • To improve the CPI, including assessment scores, assessment reports, and other results derived from the assessments, for all of our customers and respondents to our CPI assessment
  • To prevent, detect or fight fraud or illegal activities
  • To comply with legal requirements
  • For any other purpose with your consent

HOW INFORMATION IS SHARED

Care PredictorTM may disclose the information we collect pursuant to this policy, including personal information, for the following purposes:

  • To our third party service providers, such as our website operator, so they may help us operate and improve our services;
  • In the event of a merger, sale, acquisition, or other corporate transaction;
  • As required by law or law enforcement;
  • To our corporate affiliates; or
  • With your consent and in accordance with your instructions. 

By using the CPI at the request of a customer, the respondent agrees to share the information they choose to provide in their responses to the assessment as well as the assessment score(s) and assessment report(s) with both Care PredictorTM and the Care PredictorTM customer (i.e., the respondent’s employer or prospective employer). 

Care PredictorTM does not sell the personal information we collect pursuant to this Privacy Policy. 

COOKIES AND TRACKING TECHNOLOGIES

The CPI website uses cookies and may use other tracking technologies such as web beacons to help the website run, remember a user, or otherwise track visits to the website. Cookies are small pieces of information that are stored by your browser on your computer’s hard drive. While most browsers are set to accept cookies by default, you can set yours to refuse cookies or alert you before accepting them. 

The CPI website may use third parties to assist us in analyzing data regarding visitors to the website. For example, we use Google Analytics to collect information about the number of visitors to a site, the webpage that referred visitors to a site, and the pages visitors view within a site. You can opt-out of Google Analytics by downloading Google’s Opt-Out Browser Add-on.    

The CPI website may also use targeting or advertising cookies which collect information about your browsing activities in order to understand your interests for marketing purposes. We or third parties may place or recognize these cookies on your browser when you visit certain websites for the purpose of serving you with advertisements regarding our products and services that may be of interest to you. These cookies may also be used to help us regulate the advertisements you receive and measure their effectiveness.

Some internet browsers have “Do Not Track” or “DNT” features which, when turned on, send a signal to the website that the individual visiting the website does not wish to be tracked. The CPI website does not honor DNT signals, but you can adjust your cookies settings by changing the settings on your browser. 

DATA SECURITY

Care PredictorTM uses a combination of technical, administrative, and physical controls designed to maintain the security of the personal data we collect. While we take precautions designed to safeguard your personal information, no method of transmitting or storing data is completely secure.

THIRD PARTY WEBSITES

For practical reasons or for your information, the CPI website may contain links to other websites, such as links to our profile pages on social media websites. Care PredictorTM exercises no control over such other websites and is not responsible for the content thereon. This Privacy Policy does not apply to third party websites, and we recommend that you review the online privacy policy of any website you visit to determine how the operator handles personal information collected through its website.

ACCESS, CORRECTION, AND DELETION RIGHTS

If you would like to receive a copy of, make corrections to, or request deletion of the personal information we maintain about you, please contact us at the contact details listed in the CONTACT INFORMATION section below. Depending on the nature of your request, we may need to request additional information to verify your identity. 

CHILDREN’S PRIVACY

The CPI is not intended to be used by children under 18 years of age and we do not knowingly collect personal information from children. If we learn we have collected or received personal information from a child under 18 without verification of parental consent, we will delete that information. 

CHANGES TO THIS PRIVACY POLICY

We may revise this Privacy Policy from time to time. When we make changes to this policy, we will post the updated version on the CPI website and indicate the date the policy was last updated. We encourage you to review this policy periodically to check for any updates. Your continued use of the CPI after we make changes is deemed to be acceptance of those changes.
